Transcript Request Form

A transcript is a certified copy of your permanent academic record.  It contains confidential information and cannot be released without the student's specific request.  ALL information must be provided and the appropriate fees must be included.

I give OHCC permission to release my transcript to the name and address indicated above.  All holds must be cleared before an official transcript is sent.  You will be notified by e-mail if your transcript cannot be sent based on a hold.

Each transcript generated requires a fee.  After submitting this form you will receive an email which contains a link to the payment page.
